Abstract:	Wegeners granulomatosis is a rare puzzling disease generally characterized by necrotizing granulomatous lesions in the lungs and upper respiratory tract generalized necrotising vasculitis and acute glomerulitis Pathologically tehre is focal necrotizing vasculitis involving the small arteries and veins of the respiratory tract as well as those of the kidney spleen heart intestines liver mucous membranes and skin. In this rport we wish to stress that the cutaneous manifestations are often a prominent feature of wegeners granulomatosis and occasionally are the first sign of the desease. The basis for this report is II8 patients with unequivocal wegener granulomatosis screened from the word literature. The authors cheched al the reported cases of wegeners granulomatosis as well as the conditions frequently confused with it such as periarteritis nodosa lethal medline granuloma the stevens johson syndrome and several others. With but few exceptions patientes were included in this series only if an apparent diagnosis of wegeners granulomatosis was confirmed by gross autopsy examination or by microscopy biopsy examination using the widely accepted pathologic criteria of Godman and Churg (2). If there were any doubt about the diagnosis in any patient in any series or if the pathologic evidence were only presumptive that patient wasexcluded from this study. Our object was to review as many patients as possiblewith clear cut wegeners granulomatosis in order to study the extent of the cutaneos involvement in this disease. In addition to illustrate some of the problems inherent in distinguishing wegener granulomatosis from other conditions characterized by necrotizing angiitis with granuloma we have included five cases form our personal experience. All of these had mucosal lesions and four of the five had atypical pneumonias. Autopsy examination of the four who have died indicated that there had authentic wegeners granulomatosis while the evidence in the other two patients suggests their condition possibily could be due to this entity yet there are many atypical features that cast doubt on the diagnosis. Abstract:	Os autores descrevem a história de um paciente portador de hanseníase virchoviana com mais de 28 anos de duração. Ele havia feito tratamento com sulfona e recebeu alta com a baciloscopia negativa. Continuou o tratamento, mas de maneira irregular, e foi reinternado 10 anos depois com lesões cutâneas ativas, e a presença de bacilos (++) em parte típicos, nos esfregaços cutâneos. Com a suspeita de sulfono-resistência fez tratamento com clofazimina (CFZ) durante 12 meses, com regressão parcial das lesões cutâneas. Apesar de reiniciar a sulfona após esse período as lesões continuaram com aspecto regressivo, a baciloscopia apenas com bacilos granulosos, e passou a apresentar episódios eventuais de reação tipo eritema nodoso hansênico. Quatro anos antes do óbito começou a manifestar sinais de insuficiência renal cr"nica, que foi relacionada clinicamente à amiloidose sistêmica secund ria à hanseníase. A autópsia demonstrou (1) intensa impregnação de CFZ nas mucosas da vias reas superiores, g strica e intestinal, e nos linfonodos, (2) infiltrados virchovianos regressivos em múltiplas localizações com bacilos granulosos e em alguns órgãos como linfonodos, fígado e baço granulomas dimorfos com baciloscopia também positiva. 3) depósito de substância amiloide em v rios órgãos mas principalmente ao nível dos rins: 4) uma lesão nodular necrotizante próxima ao br"nquio principal do pulmão direito, evidenciando reação granulomatosa em paliçada e vasculites granulomatosas, interpretadas como manifestação pulmonar localizada de granulomatose de Wegener (GW). Tudo levou a crer que o paciente fosse portador de uma hanseníase dimorfa avançada com eritema nodoso, à qual se associou, fortuitamente ou não, a GW, seja na fase inicial ou como forma limitada, assintom tica. Segundo a bibliografia consultada, a associação de duas doenças por imunocomplexos de como são a GW e a hanseníase com eritema nodoso ainda não foi previamente documentada. Admite-se que GW possa relacionar-se alguma bactéria, ainda indeterminada, que expõe proteases neutrofílicas, gerando anticorpos neutrofílicas, anticitoplasm ticos (ANCAs). Segundo os autores a hanseníase talvez pudesse neste caso, constituir a doença bacteriana subjacente. Eles comentam também que o achado destes anticorpos na hanseníase pode sugerir a procura de manifestações da GW entre outros casos desta moléstia, que pode sugerir uma real associação entre estas duas intrigantes entidades clínicas. Abstract:	The inflammatory processes that involve the larynx can take many forms, with the granulomatous disorders invariably representing the end consequences of chronic disease. In the past, laryngeal granulomas were more prevalent, primarily because of the frequency of tuberculosis.More recently such lesions often have been diagnosed by clinicians attempting to rule out carcinoma.(AU).

Abstract:	A granulomatose de Wegener (GW) é considerada vasculite sistêmica granulomatosa necrotizante de vasos de pequeno e médio calibres, idiopática, com envolvimento multissistêmico, geralmente afetando trato respiratório e rins. Pode ser dividida em duas formas: clássica e limitada. Nesta última inexiste comprometimento renal. Os pacientes habitualmente apresentam febre, artralgias, nódulos esparsos na pele, ulceraçäo mucosa e tosse improdutiva. Os sintomas pulmonares podem variar desde sinusites até hemorragia pulmonar difusa e insuficiência respiratória. O envolvimento renal é comumente expresso pela insuficiência renal crônica (IRC). Dados laboratoriais inespecíficos refletem alteraçöes urinárias e anemia, pela IRC, e atividade inflamatória aguda. Em contraposiçäo, considera-se o ANCA o mais específico no diagnóstico. A radiografia de tórax pode mostrar infiltrados, nódulos ou cavitaçöes pulmonares. Nas biópsias observam-se vasculite neutrofílica ou granulomatosa necrotizante, com células gigantes nos granulomas epitelióides. O tratamento com corticosteróides e imunossupressores melhorou significativamente o prognóstico da doença. Uma revisäo da clínica, da histopatologia e dos achados laboratoriais da GW é apresentada, e, particularmente, as manifestaçöes cutâneas, pulmonares e renais säo discutidas. (AU).
